  • Please download, print and sign the Initial Registration Form, and review the Notice of Privacy Practices. You can find these documents under the links below. Please complete the Initial Registration Form and bring this form with you to your first appointment.
  • If you are covered by insurance, please check with your insurance company for your co-pay responsibility and possible deductible.
  • Payments are required at the time of service, preferably by check if possible. Credit cards are not accepted.
